Home With Help Homecare Services was founded by a passionate Registered Nurse and Nurse Practitioner with over 18 years experience in hospital nursing, aged care, in-home care & community nursing.

At Home With Help Homecare Services, we are passionate about providing care to older Australians and people living with disabilities, recovering from illness or accidents in our community. Our friendly, caring and experienced staff are always ready to support you so you and your loved ones have peace of mind.

We are pleased to provide various care options to our clients.

We adapt an integrated model of care approach that connects health and social services to deliver seamless, efficient and holistic care with focus on client-centred care, improved care coordination, multidisciplinary care approach, and better health outcomes

 

1. Private Care Services

We provide private in-home care services to people looking for extra help and support at home. This can either be once-off, short term (temporary) or long term (ongoing) basis.

Clients can call us to organise in-home supports and services. One of our Registered Nurses will collaborate with you to discuss your personal care needs and preferences and create a support plan. 

Private care services can benefit older Australians that are not eligible for government funding, older Australians allocated government funding but requiring more help at home, older Australians awaiting for government funding to be allocated, and adults with

Private health insurance with home care coverage requiring post hospital care at home or in the community. 

2. Support at Home (Aged Care)

We provide in-home supports and services to participants >65 years old with a Support at Home funding as an Associate provider. Participants approved and allocated Support at Home funding can choose us their main home care services provider.

We will work together with you and your registered provider to provide in-home supports and services that are tailored to your personal preferences. We will encourage you to exercise control and choice when you choose us as your service provider.

We are partners with reputable registered providers as an associated service provider. Please reach out to us to know who we partner with.

The registered provider of your choice will provide financial management, care management, manage compliance, and budget management.

We coordinate and provide services for you. You will have a choice to choose workers that best match to you to provide you with care supports or services.  

Services we provide under Support at Home Program:

   ✔️Independence Services -Personal care, Restorative care, In-home respite
   ✔️Everyday Living -Domestic assistance (cleaning, laundry), Gardening, Meal preparation, Social support and Community access
   ✔️Clinical Services -Nursing care, Allied health
   

3. Care Management Services

We have experienced Registered Nurses who can provide private care management and care coordination to assist our clients with accessing supports and services by coordinating care services to meet their personalised needs.

Our Registered Nurses will assist with

   ✔️Care planning development
   ✔️Service and Care Coordination
   ✔️Collaborating with other health care providers involved in your care
   ✔️Comprehensive nursing assessments

Care management services are suitable for self funded (private) and self managed Support at Home participants that need guidance and support by an experienced Registered Nurse.

Care management services incur an out-of-pocket fee depending on services the client needs.

4. Community Nursing Services

Our team consists of Enrolled and Registered Nurses that are skilled and experienced to provide community nursing services to clients requiring complex care in our communities.

A referral for Community Nursing Services is required and can be completed by your GP, Specialist, Nurse Practitioner, hospital discharge planners/social workers.

Our community nursing services include:

  ✔️ Stomal therapy
  ✔️ Wound care and management-Acute and Chronic
  ✔️ Medication administration & management
  ✔️ Falls management and education
  ✔️ Diabetes monitoring & management
  ✔️ Chronic disease monitoring
  ✔️ Wellbeing & Health monitoring
  ✔️ Continence assessments & management
  ✔️ Catheter care and management (IDC and SPC)
  ✔️ Injection administration
  ✔️ Compression stockings & bandages application
  ✔️ Telephone nurse advice & support

Other specialised Nursing Care including: 

  ✔️ Palliative care support
  ✔️ Dementia advice and support

5. WorkSafe and TAC Services

We work with adults recovering from motor vehicle accident or work-related injury and with approved WorkSafe and TAC claims by assisting them with supports that improve their independence as part of their recovery process.
We have caring & experienced support workers in our team with appropriate injury support training and experience who are available to assist with supports. 
We are a Worksafe and TAC (Transport Accident Commission) approved provider to provide household help services. Your GP, hospital discharge planner or Case Manager can refer you to us for services.

     ✔️Community nursing services
How Process Works
    ✔️An assessment is completed by an allied health professional or your treating GP. 
    ✔️Your treating practitioner will refer you if assessed as needing services. 
    ✔️Once your claim is assessed and approved by WorkSafe or TAC, you’ll receive an approval letter.
    ✔️Once you receive the approval letter, you or your case manager can contact us to organise your services.

We are a Registered Community Nursing Service Provider with Work Safe VIC and Transport Accident Commission (TAC). This service aims to facilitate an early and convenient discharge from hospital, acting as an alternative to in-hospital care to transition home.

We bill Work Safe and TAC directly.

6. DVA (Department of Veterans Affairs)

DVA (Department of Veterans Affairs) Household Services are provided to eligible veterans or dependants needing extra support or help at home because of a service-related injury or medical condition, recovering from surgery or have complex health issues.
These services can be provided on short term or ongoing basis dependent on your assessed need.
You may be eligible if you are:
    ✔️Unable to manage household tasks due to service-related injury or condition,
    ✔️Accepted under MRCA or DRCA claims,
    ✔️Assessed as eligible for household services
How Process Works
    ✔️Complete DVA claim form (D9319) to be assessed
    ✔️Your claim form will be assessed by DVA
    ✔️Once your claim is assessed and approved by DVA, you’ll receive a Household Services determination letter
    ✔️Once you receive the determination letter, contact us to organise your eligible household services
  
Services eligible under Household Services (HHS) program include:

    ✔️Domestic Cleaning -vacuuming, mopping, dusting, basic kitchen cleaning, washing bathrooms and toilets and surfaces

    ✔️Shopping Assistance-accompanied and unaccompanied

     ✔️Laundry Services -washing and drying, ironing, bed linen changes

     ✔️Meal Preparation Assistance 

We bill DVA directly.
